The Prisons and Probation Ombudsman today published the results of his latest post-investigation survey. The survey was introduced last year as a means to collect the views of custodial staff, healthcare professionals and coroners who had been involved in a PPO fatal incident investigation. Their feedback is essential to allow us to evaluate our performance and identify areas for improvement.
Survey results indicated recognition of the professionalism and competence of PPO’s investigators. Recommendations were widely considered by governors to be fair or very fair (95%) and clear or very clear (97%), and resulted in numerous changes to practice, which may help to save lives.
